How to Schedule an Appointment
Please call our office to schedule an appointment. Typically, your primary-care provider or dentist will refer you for an oral and maxillofacial surgery evaluation. Your provider can submit a referral online or by calling our clinic. Although we can evaluate you without a referral, many insurance plans require one in order to cover the cost of care. We can help you obtain a referral if one is needed.
