Our Approach to Pediatric Specialty Care at Swedish Issaquah Campus
A primary care pediatrician is your go-to resource to help keep your child healthy. However, if a serious problem arises, a pediatric specialist can make all the difference in your child’s diagnosis and care.

For Referring Physicians
It’s easy to refer your pediatric patients to Swedish for specialty care:
- Submit a request.
- If you use Epic: Type “SMG PEDS” next to the “Refer To” line and select the specialty or type the provider’s name.
- If you do not use Epic: Complete this form.
- Then, we schedule the appointment.
We value your partnership. We always establish a direct line of communication with you, so that you’re aware of your patient’s progress at Swedish.
You can open and print any of the following forms to complete and return to your clinic in person or by fax or mail.
For your child’s first appointment:
- Pediatric Registration
- Authorization for Disclosure of Health Information to release your records to or from another office
- Permission To Treat a Minor used when a child under age 18 goes to the doctor without a parent or guardian
- Permission to Treat a Minor When Accompanied by Non-Parent or Guardian used to designate an adult other than the parent or guardian who you authorize to give consent to medical or surgical treatment by any licensed physician or hospital
Hospital stays are stressful – especially for children – but we understand the importance of creating a supportive atmosphere. If your child needs a surgical procedure, they’ll receive this care at the Swedish First Hill Campus.
Along with critical care doctors, your team often includes hospital-based pediatricians, also called pediatric hospitalists, and over 400 staff – all trained and dedicated to working with children. They collaborate closely with all our specialty teams to ensure that your child receives the appropriate level of care, regardless of the severity of illness.
