The Swedish Pharmacotherapy Clinic is a clinical pharmacist-run outpatient clinic that provides medication therapy management to specialized patient populations. Our pharmacotherapy services include:
- Medication therapy management
- Assistance with insurance prior authorizations for medications
- Coordination with specialty pharmacies to obtain medications
Appointments are by referral only.
First visit
At your first visit, a pharmacist will:
- Review your medical history, allergies, and medications
- Assess for medication side effects and drug interactions
- Provide patient medication education
- Collaborate with your specialist provider to order your medication and assist with insurance coverage
- Schedule a follow-up visit
Follow-up visits
For follow-up visits, a pharmacist will:
- Review medication and health changes
- Evaluate efficacy and safety of therapy
- Address health concerns and triage as necessary
- Collaborate with your specialist provider to make dose adjustments as necessary
What to bring
Please bring your current insurance information and a list of all your medications including prescriptions, over the counter medicines and herbal supplements for review by the pharmacist.
