Paperwork/Information to bring to Patient Registration
To assist with the admission process, we will ask you for certain documents and information, such as:
- Social Security Number

Medicare card, if applicable

Military ID card,if applicable

Insurance card(s), including prescription card

Insurance claim forms (completed and signed), if applicable

Medicaid coupon, Healthy Options card, award letter or verification of application

Physician referral/authorization forms

Copay or deductible

Copies of Advance Directives, if desired

Complete list of personal medications (Personal Medication Record)

A note about medications
Your doctor will need a list of any medicines you normally take. Prescription drugs should not be brought to the hospital. We will provide all the medications you need during your stay.
Smoke-free environment
All Swedish locations are smoke-free. This means smoking is not allowed on Swedish property, owned or leased, including parking garages and lots.
