Our Advanced Cardiac Support Program provides comprehensive care for people living with heart failure, from newly diagnosed patients to those with advanced disease. Our multidisciplinary team works closely with patients and referring providers to optimize medications, monitor symptoms and evaluate advanced treatment options when needed. We focus on helping patients improve quality of life reduce hospitalizations and receive the right care at the right time.

For some patients with severe heart or lung failure, advanced support may be needed to help the body recover or bridge to the next stage of care. ECMO, or extracorporeal membrane oxygenation, is a form of temporary heart-lung support used in highly specialized settings. The Advanced Cardiac Support Program is recognized as an ELSO Gold Center of Excellence for ECMO. The honor reflects the high-quality critical care provided, with survival rates that exceed the national average.

Some patients with Class IV Heart Failure may benefit from advanced therapies like left ventricular assist devices (also known as LVAD) or heart transplant. Our physicians have advanced training in LVAD and transplant; our nationally accredited LVAD Program consistently achieves above average survival, and our experienced physicians have successfully partnered with local and national transplant programs throughout the years. The program has a higher than national average survival for LVAD and lower rates of comorbidities.

Our team has expertise in diagnosing and managing complex heart muscle disorders and infiltrative heart diseases, including cardiac sarcoid, cardiac amyloid, genetic cardiomyopathies and hypertrophic cardiomyopathy. These conditions often require specialized evaluation, advanced imaging and coordinated long-term management. We work with patients and referring providers to develop personalized treatment plans and connect patients with the specialized resources they may need. 

Remote Patient Monitoring helps our care team stay connected with patients between visits. Through ongoing monitoring and proactive outreach, we can identify changes in a patient's condition sooner, support treatment adherence and help prevent unnecessary hospitalizations. This additional layer of support allows patients with heart failure to receive more connected, personalized care while remaining in their home environment.
