New Patients

Medical

If you are new to our clinic and would like to see a doctor, please follow these steps:

  • See a primary care physician at any of the accepted family medicine clinics to determine if you need to see a medical specialist.
  • Ask the primary care physician to fax your medical referral form, supporting chart notes, imaging/lab results (if completed), and Project Access Northwest enrollment form to Project Access Northwest directly at 206-382-3507.
  • Call Project Access Northwest at 206-788-4559 for questions or want to confirm that your referral was received. Project Access Northwest will link you to the appropriate specialist and provide follow-up case management services.
