All patients are screened for fall risk level at admission. Look to see if there is a flyer posted at patient rooms indicating that a patient may be at risk for falls. Never attempt to assist a patient to navigate within a room or exam room – notify a provider for assistance. If you find a patient has fallen, do not attempt to lift or move them – call for help and notify a provider immediately.
All Inpatients Should Have The Following:
- Call light within reach.
- Bed in lowest position with brakes locked.
- Clear environment.
- Crib rails or bed rails in highest position for pediatrics.
High fall risk interventions include:
- Yellow socks for adults
- Orange or green for toddlers/youth
- Purposeful rounding
- Bed or chair alarm or other assistive device
