What makes Swedish Family Medicine - Ballard unique?
Your personal physician at Swedish Family Medicine - Ballard has fewer patients in his or her practice than traditional primary-care doctors. This means that you will have 30-60 minutes with your doctor to address your concerns in a comprehensive manner. Your physician also has time to provide the preventive health care that keeps you well. When a medical need arises, you can conveniently get a same-day or next-day appointment.

During clinic hours, a staff member will answer the phone promptly. Through e-mail, you can discuss your medical care directly with your health care team and receive a timely response. After hours, you can phone a physician for urgent issues.

If you need more complex care, our health care team will use their experience and resources to help you find the best specialists. We will continue to coordinate your care with these specialists.

How do I get to the clinic?

How do I pay for my care at Swedish Family Medicine - Ballard?
There are two ways to pay for your care at our clinic.
1. Direct enrollment. Whether you are uninsured, underinsured or have other health insurance, you can pay us an affordable monthly fee directly. You will be billed for the previous month's enrollment fee at the beginning of each month.
2. Through an insurance carrier. If you are covered by Medicaid, Medicare or certain commercial insurers, you may be able to enroll in a plan through your insurance company. If you enroll through your insurance plan, there will be no monthly fee, co-pays or co-insurance fees for any of the services that we provide at our clinic.

What does my monthly fee cover?
Your monthly fee covers unlimited access to your health care team and any services that we provide in our clinic. (See Guide to Patient Services.)

Are services received outside of the clinic covered by my monthly fee?
No. Your monthly fee covers only the services outlined in our Guide to Patient Services. If you are hospitalized, receive any outside tests or see any specialists, whether we referred for those services or not, you or your insurance company will be billed separately by the lab, specialist or hospital. 

How will I know if you add or discontinue services?
We will notify you if we change any services that are currently included in your monthly fee.

If I already have health insurance, can I enroll as a patient in your clinic?
Yes. If you are covered by Medicaid, Medicare or private insurance, you may be able to enroll in a plan that covers our services. If you have insurance that does not participate in our program, please talk to our staff about your options. 

Can I enroll my child if I am not a patient myself?
Yes. She or he can be enrolled through one of the insurance companies we work with or directly with us. 

Can I choose the physician who will provide my care?
Yes. We want you to feel totally comfortable with your physician. Our physician staff consist of family medicine faculty and resident physicians. You will see your personal physician whenever possible. If you require urgent care and your physician is not available, another physician will see you. All of our physicians will have access to your medical records. You are welcome to change physicians if you wish.

How do I get in touch with my doctor?
If you would like to talk to a member of your health care team, please call us at 206-297-5100 during clinic hours. Your call will be answered promptly by a staff member. After hours, you may call and leave a voice message for any non-urgent needs (e.g., appointment requests, prescription refills.) All voice messages will be returned by the next working day by someone on your health care team. For urgent questions after hours, you will be connected  to the hospital operator who will get you in touch with an on-call physician. 

You can also e-mail your health care team directly at any time to ask clinical questions, request perscription refills and discuss test results. E-mails are returned by the next working day. Please do not send urgent requests through e-mail - call our office and speak to someone directly.

Will I have access to my medical record?
Yes. Our patient privacy policy states that you are entitled to access and may receive a copy of your medical record at any time. If you would like to do so, please contact our office.

Do you make house calls?
No, we do not.

What should I do if I have an emergency?
If you suspect a life threatening emergency, call 911 immediately. If you have an urgent, non-life threatening condition - a minor cut, infection or acute illness - call our office to speak with a member of our health care team. If you do not need emergency care, we will see you in the clinic at the earliest opportunity.

Please note that your monthly fee does not cover the costs of ambulance transportation, emergency room care, hospital services or follow-up specialty care. Please review your insurance plan so you'll know ahead of time which costs are covered. If you do not have insurance, we encourage you to obtain catastrophic insurance to cover such emergencies.

What if I need hospital care?
One of the benefits of joining our clinic is being part of Swedish Medical Center, the largest nonprofit health-care institution in the Greater Seattle area. We are very familiar with - and have every confidence in - Swedish's physicians, technology and facilities.

If you need hospital care, we will arrange for your admission to Swedish's First Hill, Cherry Hill or Ballard campus, or to another appropriate facility. Your physician will communicate closely with the hospital team responsible for your care and may even be a member of that team. Having your medical record already at Swedish will help facilitate your care. If you are admitted to a hospital other than Swedish, your Swedish Family Medicine - Ballard physician may not be able to directly coordinate your care. However, we will try to be in contact with the physician caring for you. Upon your discharge from the hospital, we will help you manage your follow-up care.

What if I need specialty care?
At times, you or your physician may feel that you would benefit from specialty consultations. In these cases, we will refer you to an appropriate specialist and work closely with him or her to provide your ongoing care. While we know Swedish specialists bests, our goal is to find the best possible specialist for you, no matter where he or she practices. Any specialty care will be billed separately and you or your insurance will be responsible for covering the cost.

If I become a patient at your clinic, can I still see my gynecologist?
Yes. All of our physicians provide women's health care, including annual exams, contraceptive counseling, IUD placement and pregnancy care. However, we understand that many women value the long-term relationships they have with their gynecologists and may wish to continue their care there. You or your insurance will be billed separately by your gynecologist.
