Refer a Patient

How to Refer Your Pediatric Patients to Swedish

It just takes two steps:

  1. Submit a Request
    • Complete the Referral Form and fax to the corresponding number on the form.
    • If you are on Epic: Type in “SMG PEDS” next to the “Refer To” line and select the specialty or simply type in the physician’s name.
  2. We Schedule the Appointment

We’ll take it from here and contact your patient directly to schedule an appointment. Although it depends on the case and type of specialty, most patients are seen within a week.*

We value the trust you place in us when you refer patients to Swedish; therefore, we establish a direct line of communication with you to keep you apprised of your patient’s progress.

*Appointment schedules vary for the following specialties: endocrinology, epilepsy, nephrology, nutrition and urology. In some cases, our specialists will contact the referring physician first prior to scheduling a patient appointment.
