To educate health care providers on how to comfortably communicate the importance of advance care planning by being able to identify pitfalls and limitations and describe the planning process and roles of patient, proxy, clinician and others to the patient.
This online educational program is intended for primary-care providers, fellows and residents.
At the conclusion of this course, participants will be able to provide better patient care through an increased ability to:
Define advance care planning and explain its importance
Describe the steps of the advance care planning process
Describe the role of the patient, proxy, clinician and others
Identify pitfalls and limitations in advance care planning
