At Swedish Visiting Nurse Services, our goal is to provide you or your loved one the assistance to remain in the home. We're committed to promoting the highest level of patient independence. We ensure that family members are very involved in the plan of care. In addition to the broad range of services available to patients, support services are also available for families and caregivers.
This fully licensed Medicare/Medicaid certified program works closely with physicians and other Swedish programs to ensure continuity of care and ease a patient's transition from hospital to home.
