Mapping a Course for Comfort and Healing with Palliative Care
This program is for patients diagnosed with a serious, life-threatening illness, often with an unknown treatment outcome. While undergoing treatments, the Pathways team can support patients in many ways such as:
- Symptom management to provide comfort and enhance quality of life
- Care coordination with the patient's medical team
- Community resources to address financial and practical needs
- Spiritual and emotional support
- Rehabilitative or personal care services needed during this time

Special Care at a Special Time
Many of the services provided to our Home Health and Pathways patients are included in the care we provide to those experiencing a life-threatening illness who are not receiving curative treatment. As part of this dedicated level of care, hospice patients benefit from comprehensive services personalized to promote comfort and quality of life, including:
- Care coordination with the patient's primary physician
- Pain and symptom management
- Rehabilitation therapy consultations
- Resource coordination to address financial and practical needs
- Medication, medical supplies and equipment related to the hospice diagnosis
- TeleHealth
- Personal care
- Volunteers to support patients and caregivers
- Comfort therapies such as massage, music and acupuncture
- Spiritual and emotional support
