Healthy Hearts is a cardiac-rehabilitation and risk-reduction program for people who have had a heart attack, stent placement, bypass surgery, valve surgery, or who have stable angina (chest pain). The purpose of this program is to partner with participants and their physicians to assist each individual in achieving their health-care goals.
About the program
The program consists of two key components. Medically supervised, on-site exercise sessions are individualized and designed to gradually increase each participant’s strength, endurance and confidence. These sessions last from two to 12 weeks.
Healthy Hearts also includes a year of risk-reduction education, with individualized coaching sessions. Participants can take part in the sessions on site, at Swedish or at home via telephone or computer. Education topics – ranging from nutrition to smoking cessation to stress management – address each individual’s medical history and readiness for change.
Healthy Hearts services include:
• A one-on-one health evaluation
• A customized exercise plan
• Personalized risk-reduction education
• Individual instruction about safe exercise practices
• Development of a home exercise program
• A baseline measurement of health at the start of the program, with follow-up measurements at 12 weeks and one year
• A multidisciplinary staff of experienced health-care professionals
Patients with known coronary artery disease and cardiovascular disease are eligible to participate in this program. Insurance coverage will vary.
