The Transfer Center offers:
- Streamlined patient transfers
- Seven days a week - 24 hours a day
- All acute-care specialties
- Admissions to Ballard, Cherry Hill and First Hill campuses
The Transfer Center staff quickly connects referring physicians with Swedish physicians to secure the appropriate bed. Whether a patient is in another hospital, emergency department or medical office, the process is simple:
- Referring physician calls the number above
- The Transfer Center's registered nurse collects the following information from the referring physician:
- Referring hospital and/or physician name and phone number
- Patient's name, age, diagnosis, clinical status and type of specialty care or service requested
- Patient's primary care provider (PCP) name and phone number
- The Transfer Center nurse facilitates a physician-to-physician phone consult between the referring and admitting physicians. The nurse will attempt to remain on the phone to complete the transfer after the patient has been accepted.
- The Transfer Center nurse uses Interqual Guidelines to discuss and evaluate symptoms with the admitting physician to assign an appropriate bed. The Transfer Center assists with transfer details and collects additional information needed prior to transfer.
- After the call, the Transfer Center staff communicates clinical information to the receiving unit staff. If connected to the Swedish EPIC system, the admitting physician can enter admission orders in the electronic medical record to expedite care on admission.
