System Director of Pharmacy

David R. West, RPh, graduated from the University of Washington in 1979. He has been the System Director of Pharmacy since 1997, overseeing all five locations as part of the pharmacy management team. He has been at Swedish since 1976. He is an active member in the American Society of Health-System Pharmacists and the Washington State Pharmacist Association. dave.west@swedish.org

Residency Program Director

Roger S. Yamaguchi, PharmD, graduated from the University of California, San Francisco in 1986. He completed a PGY1 residency at Franciscan Health System – St. Joseph Hospital in Tacoma, WA. He has been at Swedish since 1990, and currently is an Oncology pharmacist on the First Hill location. He has been an active member of the American Society of Health System Pharmacists. He is a Clinical Assistant Professor of Pharmacy at the University of Washington, University of North Carolina, and Midwestern University Schools of Pharmacy. He has been the lead preceptor since the program’s inception in 2003 and Residency Program Director since 2010. roger.yamaguchi@swedish.org

Residency Program Coordinator

Kris D. Eng, RPh, graduated from the University of Washington in 1993. She completed a PGY1 residency at Valley Medical Center. She has been at Swedish since 1999 and currently works as a Critical Care Clinician on the First Hill location. She is an active member in the American Society of Health-System Pharmacists and the Seattle Area Society of Health-System Pharmacists. She has been a Clinical Assistant Professor at the UW School of Pharmacy since 2000. With former Residency Program Director, Paul Anderson, Kris co-created the pharmacy practice residency program at Swedish. She has been the Residency Program Coordinator since its inception in 2003 . kristine.eng@swedish.org

Director, System-wide Clinical Pharmacy

John E. Zarek RPh is a graduate of the University of Illinois College of Pharmacy in 1975. He previously obtained a BS in Biology in 1971 from John Carroll University. In 1988 he obtained a Certificate in Geriatric Pharmacy Practice from the University of Washington. He has been a member of the American Society of Health-System Pharmacists since 1975. In 1984 he was named Outstanding Hospital Pharmacist of the Year from the Washington State Society of Hospital Pharmacists. He has been a preceptor since 1982. He has presented on numerous occasions both clinical sessions and poster sessions at Midyear Clinical Meetings. He has extensive expertise in the areas of adverse drug reactions and medication misadventures with leadership responsibilities. He has provided leadership in multiple aspects of patient safety within the hospital setting that includes addressing organizational culture. Extensive Medical and Nursing staff collaboration on a wide variety of patient care pathway teams has resulted in numerous quality improvement initiatives. john.zarek@swedish.org
