Application and Interview Process
 

All applicants must participate in the ASHP Resident Matching Program.

If you are interested in becoming a resident at Swedish's Pharmacy Practice Residency Program, please apply as follows:
 

A. Register for the ASHP Resident Matching Program by January 15.

B. Submit all application materials via email consisting of:
 

  1. Curriculum vitae (no longer than 10 pages)

    a. Please include any previous names used in official correspondence

    b. Please title your document: "Last name, first name - CV"

    c. Please see "Frequently asked questions - Application Process" for details on content.

  2. Letter of intent (one page only)

    a. Please title your document: "Last name, first name - LOI"

    b. Please see "Frequently asked questions - Application Process" for details on content.

    c. Please address letter of intent to "Resident Selection Committee

    d. Personal statements will not be accepted

  3. Three written letters of recommendation along with our standard letter of recommendation form (click to open file)

    a. One must be from a rotation preceptor
     
    b.
    One from an employer and/or academic advisor

    c. Letters must be submitted on formal letterhead and not within the content of an email 


    d. Have references submit letters of recommendation via email to kristine.eng@swedish.org

    Our standard form must accompany a written recommendation letter. We cannot accept ASHP letter of recommendation forms. Recommendation letters are due January 10th

  4. Official copy of college transcript for pharmacy school only

    Mail pharmacy transcript to:

C. Submit curriculum vitae and letter of intent to Residency Program Coordinator via email to:

pharmacyresidencyprogram@swedish.org

     1. Please include in the email subject line: "Last Name, First Name - Name of School of Pharmacy"

     2. All materials must be received by January 15th, 11:59 PM PST in order to be eligible for consideration for an interview.

Please note that mailed submissions will not be accepted.
 

D. If selected, complete an on-site interview on the following dates only: Wednesday February 8, Friday February 10, Monday February 13, Wednesday February 15 and Friday, February 17. Applicants selected for an interview will be contacted by January 31.

 E. Submit ranking list by Friday, March 9 to National Matching Program Web Site. The SMC ASHP Match number is #154413. Results of the match will be released on Wednesday, March 21.
