How to Apply to volunteer at Ballard

To apply to be a volunteer at Swedish's Ballard campus, please print and fill out the following forms:

• Cover Sheet
• Volunteer Application
• Volunteer Memo/Agreement
• Confidentiality Agreement
• Disclosure Form
• Washington State Patrol Background Check (complete section C and return the completed form to Swedish with your volunteer application forms)
• Health Questionnaire (include copies of the immunizations requested on the Cover Sheet)
• Reference Forms (we require TWO references from people who are not related to you.  Please print out two copies of the reference form.  Your references may be returned separately or with the rest of  your completed application.
