Premium Holiday Pay

All hourly employees

Extended Illness Bank (EIB)

3.33 hours per month (pro-rated for part time).  Maximum accrual is 1,040 hours.  Can be used for your own or your family member’s injury or illness lasting 4 or more days.

Basic Life Insurance

All employees working .5 FTE and above

Benefit equal to one time annual salary, rounded to next $10,000.  $500,000 maximum without submission of evidence of insurability.

Basic LTD

Benefit equal to 50% of salary ($1,000/month max.) after a 180-day waiting period.

Benefit equal to percentage of the AD&D Insurance Benefit in effect and is determined by the loss suffered.

Retirement

401(k) – Employer 5% base contribution vested 100% after five years. 100% employer match on up to 2% of yearly salary – vested immediately. (Subject to annual IRS limits)

Set aside earnings pre-tax for retirement. Subject to annual IRS limits.

Medical coverage, including hospitalization and preventive care.  Choices include:

1.             Swedish Advantage (PPO)

2.             Swedish Standard (PPO)

3.     Consumer Choice Plan

4.             Waive Coverage – monthly credit  (must have other group medical coverage)

Dental Insurance

All benefits-eligible employees scheduled to work at least .5 FTE and above, and their eligible dependents

Dental coverage including preventive, basic and major services.  Choices include:

1.             Swedish Dental (Washington Dental Service)

2.             Waive Coverage

Vision Insurance

All benefits-eligible employees scheduled to work at least .5 FTE and above, and their eligible dependents

Includes benefit for annual eye exam and the purchase of hardware (glasses or contacts - $250 / two years).  Coverage is provided through participation in any of the Swedish medical plans.

First day of the month following employment unless hire date is first working day of the month

Supplemental Employee Life Insurance

All benefits-eligible employees scheduled to work at least .5 FTE and above

Group term life insurance for employees.  Can purchase insurance in increments of $10,000 up to 6X annual salary. $350,000 Guarantee Issue.

Dependent Life Insurance

 

All benefits-eligible employees scheduled to work at least .5 FTE and above

 

 

Group term life insurance for dependents.  Can purchase insurance for child(ren) of $10,000 and ½ employee’s election for spouse, with $50,000 Guarantee Issue.

Long Term Disability (LTD)

All benefits-eligible employees scheduled to work at least .5 FTE and above

Option to buy up from company-provided LTD benefit.  Choices include:

1.        60% of salary ($10,000/month max.) after a 180-day         waiting period

2.        60% of salary ($10,000/month max.) after a 90-day waiting period

Bus Pass Subsidy

50% subsidy of monthly bus pass

Allows for up to 3 days time off (5 days for extensive travel) in the event of a death of an immediate family member.

Tuition Reimbursement

Benefit equals 50% of tuition only up to $1000 per calendar year.  Can be used for college/university classes, pass/fail courses, and certificate programs (subject to pre-approval).
